I want to Download My Records
This is the fastest and easiest option. You'll be notified when your records are ready and you can download them immediately.
I want My Records on CD
You can order a physical copy of your records on CD. You'll be notified when they are ready for pickup, or you can choose to have them mailed to you.
I want My Records Sent to Someone Else
You can request your medical records be sent to someone else such as your doctor for continuing care.
